Sacrament Records Request

To protect the privacy of the individual involved certificates are only issued to the person whose record is requested, or a person who provides proof of parental authority, guardianship, power of attorney, executor/personal representative or other legal status on behalf of the person whose record is requested. NO official certificates are issued for genealogical purposes. Photo ID must be presented at the time of request. Turn-around time is approximately 1 week. Church staff will contact you when your certificate is ready for pick-up.
